Content | Dear Students, To save your precious time, please prepare the following documents as listed below for Check-In: 1. Please follow the instructions on the acceptance admission notice. You may refer to the attached document for details. 2. Prior to check-in, please be sure to log in to the following website to register basic information and to upload a photo for student ID. Please also print out the “Student Basic Information Sheet”, the “Comprehensive Data Sheet”, and the “Military Status Questionnaire” respectively and hand them in to the check-in department. http://campus4.ncku.edu.tw/wwwmenu/program/net/checkin/netfreshman.html 3. When checking in, please bring your student ID or your original form of identity card for inspection, and submit the following documents: 1) New Student Basic Information Form(Please affix copies[both sides] of your identity card to the form) 2) Integrative Information Sheet 3) Professor’s Consent Form(Turn in within one month after check-in. Students in Power Group must also submit the new student interview progress form) 4) Graduate diploma (Please bring the original certification and submit a copy of it. Students who have not yet obtained a diploma can check in first and fill out the “AffidavitLetter”. Bring the original certification and submit a copy of it for inspection before the first day of the semester.) 5) Military Service Questionnaire(Submit to Yunping Building Student Assistance Division. Female Students excluded) ※Please submit documents 1-4 in order, and note your student ID number and Group on the upper right corner of the New Student Basic Information Form. 4. Time of Check-in: 2017.05.31 9~11am and 2~4pm Please bring all the required documents and credentials listed above to Tzu-Chiang Campus, 4th floor of the Electrical Engineering Department Building to proceed the check-in process. 5. It is recommended to have a professor prior to check-in(please contact professors from each group to schedule meeting times). Students must find a professor within one month after check-in, and submit the professor consent form. 6. Those who asked for a commissioner to check-in, please attach the Letter of Authorization and submit the aforementioned documents and the identity card of the trustee upon inspection. 7. Those who gave up on check-in, please send the “Withdraw Admission Form” to 8. Those who failed to check-in on time will lose their qualification of admission, and the vacancy will be filled by other students on the waiting list. 9.
